It is a problem for American workers and has been for quite a while. It is one of the reasons I chose to retire in 2015. Wages have been undercut and older workers have been displaced because of high salaries. Fortunately, the offshoring of IT services entirely began to come back onshore because of poor performance and difficult project management. Those promises to work in the boonies turn out to be surprisingly difficult to enforce. Young people want to live in cities and the medical associations are not willing to give sufficient bonuses (which would come from the pay packets of doctors in cities) to make the outback attractive. In my province there is a clear segregating tendency in the medical workforce: younger, urban, Canadian females; older, rural, foreign males.
