They call it an incongruence. That means there is a disconnect between the reality of one's identity and the perception. 

Per the Cleveland Clinic,  A mental health disorder, also known as a mental illness or psychiatric disorder, is a condition that affects the way you think and behave.

Does gender identity disorder affect the way the individuals think or behaves? Yes. It is a lie to say that trans is not a mental disorder. DSM-5, the Cleveland Clinic and sane people every where agree.

The only real fad surrounding transexuals was the outbreak of a really virulent right wing overreaction to them. Overreacting like that has always been trendy thru the ages though.

Two things can be true. It can be true that there are legitimate cases that exist in history and even today. It can also be true that the vast vast majority of the cases that we see today are a result of it being trendy and not actual transgenderism
