The authors of that study cite anxiety, among other possible factors, leading to increased erections. Combine that with the incredibly small and skewed sample size and highly questionable methodology, this study is only ever cited positively in a political context and is almost always cited negatively in a scientific context. The point of the statement was that living conditions for blacks in democratic cities are worse than anywhere else in the country. It is probably a little bit of hyperbole to say third world.
